TNT Empowerment Network offers Community Health Worker (CHW) services to support individuals and families as they navigate health, wellness, and essential community resources. These services focus on education, coordination, and access to care through trusted, community-based support. TNT Empowerment Network provides community-based support and education services only and does not provide medical services.
CHW Services Include:
- Care coordination and referrals
- Health education and healthcare system navigation
- Appointment support and follow-up
- Resource linkage, including ForwardHealth, benefits, clinics, and community programs
- Family and maternal support
- Community outreach and advocacy
